How to Write a Compounded Prescription

Writing a compounded prescription is very similar to a standard medication. The most complex part is choosing the active pharmaceutical ingredient(s) (API) and the dosage form. Please visit our Compounded Specialties page or call us if you have questions.

Please include the following information:

​

  • Patient name and date of birth

  • Patient phone number

  • Active drug name(s)

  • Dose/Strength/Concentration of each drug

  • Dosage Form (i.e. Oral Tablet, Vaginal Cream, Rectal Suppository, etc.)

  • Directions for use

  • Quantity to be dispensed

  • Number of refills authorized

  • Contact information for prescriber
